Child Health Partnership is seeking a full-time Registered Nurse who is interested in in-home community health and education. As a Child Health Partnership Community Nurse, you will have the opportunity to build and cultivate empowering relationships with parents and children in their home environment. This position will allow you to work in coordination with a Family Support Specialist to serve low-income families with children ages 0-6 years old, pregnant women and teens, as part of an established in-home visiting program. Child Health Partnership is looking for a nurse who can provide health assessments, education, and improved access to care, while working in collaboration with community resource agencies to promote family self-sufficiency. The ideal candidate will have experience with children and families, community resources and good interpersonal skills. A current state of Virginia RN license and a professional CPR certification are required, BSN is preferred. Home-based or community health experience, and a willingness to work in a team environment desired. Bilingual English/Spanish is a plus. Child Health Partnership has a flexible Monday to Friday schedule, excellent benefits, and starting salary of $28+ per hour, based on experience. Must have a reliable vehicle (mileage reimbursed). Exciting full-time opportunity to work with a professional team in a trusted home visiting program serving local families for over 30 years. Child Health Partnership seeks a Family Support Specialist/Parent Educator to encourage optimal health and well-being for pregnant women, children 0-6 and their families. This position partners with parents to promote positive child development and school readiness. Associate Degree and experience in social work, child development, or related field required. Bachelor’s Degree and Spanish proficiency preferred. Must have reliable vehicle (mileage reimbursed). Salary range starts at $38,000+ based on experience with excellent health, vacation and sick leave benefits. Bilingual Mental Health Counselor – Child Health Partnership, a community-health, home-visiting program in Charlottesville, VA, seeks a bilingual Mental Health Counselor to provide in-home support for parents with children 0-6 years of age, who are experiencing depression, anxiety and other mental health challenges.

Essential duties include:

  • Work closely with Child Health Partnership staff to identify current families’ needs for in-home, community-based counseling sessions.
  • Work with parents to develop and implement individualized treatment plans.
  • Provide individual counseling sessions to address and help parents achieve identified goals.
  • Document all treatment activities and communications.
  • Maintain consistent communication with staff and community partners as needed.

Qualifications:

  • MSW or LPC required; license preferred
  • Proficiency in English and Spanish
  • Minimum one year of clinical work experience
  • Experience with families or children preferred

Child Health Partnership offers flexible schedule (some late afternoon/early evening hours may be needed) and will consider part-time or full-time with benefits.
